Dengue Fever Vaccination

Dengue fever, a viral illness transmitted primarily by Aedes mosquitoes, is a growing global health concern. With an estimated 390 million infections annually, it poses a significant threat to human populations in tropical and subtropical regions. This arbovirus, belonging to the Flaviviridae family, is characterized by sudden onset of fever, severe joint and muscle pain, rash, and other flu-like symptoms.

🦟 What is Dengue Fever?

Dengue fever is a viral illness transmitted by Aedes mosquitoes, particularly Aedes aegypti and Aedes albopictus. The virus belongs to the Flaviviridae family with four distinct serotypes (DENV-1, DENV-2, DENV-3, DENV-4). Infection with one serotype provides lifelong immunity to that specific type but only temporary protection against others.

⚠️ How It Spreads

  • Bite from infected Aedes mosquitoes
  • Mosquitoes active during daytime (early morning & late afternoon)
  • Cannot spread directly person-to-person
  • Mosquitoes breed in standing water
  • Common in tropical & subtropical regions

👥 Who's at Risk?

  • Travelers to endemic regions (Southeast Asia, Caribbean, Central/South America)
  • Residents of tropical/subtropical areas
  • Individuals without prior exposure to dengue
  • Pregnant women (higher risk of complications)
  • People with chronic medical conditions

Vaccination Schedule (Qdenga Vaccine)

Age Dose Required Schedule Time Before Travel Boost Required At
4 years and over 2 0 & 3 months 2 weeks or more -

Qdenga is now licensed and approved in the UK, offering protection against all four dengue serotypes.

🩺 Common Symptoms

  • Sudden high fever (up to 104°F/40°C)
  • Severe headaches
  • Intense joint and muscle pain ("breakbone fever")
  • Pain behind the eyes
  • Characteristic skin rash
  • Nausea and vomiting
  • Fatigue and weakness
  • Mild bleeding (gums or nose)

🚨 Severe Complications

Warning signs requiring immediate medical attention:

  • Dengue Hemorrhagic Fever (DHF): Severe bleeding, blood plasma leakage
  • Dengue Shock Syndrome (DSS): Dangerously low blood pressure, organ failure
  • Severe abdominal pain
  • Persistent vomiting
  • Rapid breathing
  • Bleeding gums or nose
  • Blood in vomit or stool

⚠️ Severe dengue can be life-threatening. Seek immediate medical care if warning signs appear.

🔄 Four Dengue Serotypes

Understanding serotype immunity:

  • DENV-1, DENV-2, DENV-3, DENV-4: Four distinct virus types
  • Infection with one serotype = lifelong immunity to that type
  • Subsequent infection with different serotype = higher risk of severe dengue
  • Antibody-dependent enhancement (ADE) increases severity
  • Qdenga vaccine protects against all four serotypes

🔄 Dengue Transmission Cycle

The dengue virus follows a specific transmission cycle:

  1. Mosquito Infection: Female Aedes mosquito bites an infected person during their viremic period (when virus is in bloodstream)
  2. Virus Incubation in Mosquito: Virus replicates within the mosquito for 8-12 days
  3. Human Transmission: Infected mosquito bites another person, injecting the virus into their bloodstream
  4. Incubation in Human: Incubation period of 4-10 days before symptoms appear
  5. Viremic Period: Infected person can transmit virus to other mosquitoes for 4-5 days

Important: Dengue cannot be transmitted directly from person to person. The mosquito is essential for the transmission cycle.

💊 Treatment Approach

Currently no specific antiviral treatment for dengue:

  • Rest and hydration are essential
  • Pain relievers (acetaminophen/paracetamol)
  • Avoid aspirin and NSAIDs (can increase bleeding risk)
  • Monitor for warning signs of severe dengue
  • Hospitalization may be needed for severe cases
  • IV fluids for plasma leakage

💉 Qdenga Vaccine

New vaccine now available in the UK:

  • Licensed and approved for use
  • Protects against all four dengue serotypes
  • Two-dose schedule (0 & 3 months)
  • Suitable for ages 4 years and older
  • Addresses antibody-dependent enhancement concerns
  • Effective for those with or without prior dengue exposure

🛡️ Prevention Measures

  • Vaccination: Get Qdenga vaccine before travel
  • Mosquito repellent: Use DEET-based products
  • Protective clothing: Long sleeves and pants
  • Mosquito nets: Sleep under treated nets
  • Remove standing water: Eliminate mosquito breeding sites
  • Peak hours: Stay indoors during early morning/late afternoon
  • Window screens: Keep mosquitoes out of living areas

🌍 Endemic Regions

Dengue is prevalent in over 100 countries across tropical and subtropical regions:

Southeast Asia: Thailand, Vietnam, Singapore, Indonesia, Philippines, Malaysia

South Asia: India, Sri Lanka, Bangladesh, Pakistan

Caribbean: Puerto Rico, Dominican Republic, Cuba, Jamaica

Central America: Mexico, Guatemala, Honduras, Nicaragua, Costa Rica

South America: Brazil, Colombia, Venezuela, Ecuador, Peru

Pacific Islands: Fiji, French Polynesia, New Caledonia
