Yellow Fever Vaccination

Yellow fever (YF) is a vaccine-preventable viral infection transmitted predominantly by certain species of day-biting mosquitoes. It is prevalent in tropical & sub-tropical regions of Africa and Central & South America. Outbreaks have caused up to a 50% mortality rate. The World Health Organisation (WHO) recommends vaccination if travelling to risk areas. Some countries require a certificate of proof of vaccination to enter. An International Certificate of Vaccination or Prophylaxis (ICVP) must be obtained from a registered yellow fever centre.

🦟 What is Yellow Fever?

Yellow fever is a viral hemorrhagic disease transmitted by infected mosquitoes, particularly the Aedes aegypti species. The virus is endemic to tropical regions of Africa and Central & South America. The disease gets its name from the jaundice (yellowing of skin and eyes) that affects some patients.

Important: Yellow fever vaccine provides lifelong immunity for most people. A single dose is usually sufficient. The International Certificate of Vaccination or Prophylaxis (ICVP) is provided and valid for life.

Acute Phase (3-4 days):

  • Sudden onset of fever exceeding 38.5°C (101.3°F)
  • Intense headache and back pain
  • Chills, shivering, and muscle aches

  • Dizziness and light-headedness
  • Fatigue and weakness

Toxic Phase (Severe - 15% of cases):

  • Jaundice (yellowing of skin and eyes)

  • Bleeding from mouth, nose, eyes, stomach
  • Liver and kidney dysfunction
  • Delirium, seizures, and confusion

🦟 How It Spreads

Primary transmission:

  • Mosquito bites: Primarily Aedes aegypti (day-biting species)
  • Sylvatic cycle: Mosquitoes feed on infected monkeys in jungle areas
  • Urban cycle: Mosquitoes transmit between humans in populated areas
  • Peak activity: Early morning and late afternoon
  • Incubation period: 3-6 days after mosquito bite

NOT transmitted through:

  • Casual contact (touching, kissing)
  • Respiratory droplets (coughing, sneezing)
  • Food or water

Note: Rare human-to-human transmission can occur through blood contact in severe cases.

Unvaccinated Travelers:

Anyone traveling to endemic regions without vaccination faces significant risk. Yellow fever has a 50% mortality rate in severe cases.

Occupational Exposure:

Forestry workers, researchers, laboratory personnel, and outdoor workers in endemic areas have increased exposure to infected mosquitoes.

Older Adults:

Individuals over 60 have higher risk of developing severe disease and complications, including organ failure.

Immunocompromised:

Those with weakened immune systems may not be able to receive the live vaccine and should consult their doctor.

  • Fever management: Paracetamol (avoid aspirin/NSAIDs due to bleeding risk)
  • Organ support: Dialysis for kidney failure if needed
  • Blood transfusions: For severe bleeding
  • Rest: Complete bed rest during acute phase
